Form 116 - Notice of intended application for probate, administration or reseal (information about how you publish your own notice online. NOTE: you must publish this notice using the online publication tool. The PDF version and Word versions of this form only contain the prescribed wording of the notice that will be generated once you complete the online form).
NOTE: From 30 April 2012, all affidavits are required to include a certificate by the person witnessing the affidavit that they have identified the person swearing or affirming the affidavit. The precedent forms above include a form of certificate that the witness can use. If the affidavit is sworn before 30 April 2012 this certificate can be deleted.
